New Patient Registration

registrationShould you live within our practice area and wish to register with our practice you will need to complete either a Registration form or your NHS Medical Card and a New Patient Questionnaire. 

Please hand into reception with some identification (photo ID if possible) before making an appointment.

We accept patients from Newquay and the surrounding areas including Crantock, Cubert, Holywell Bay, Newlyn East, Watergate Bay and Mawgan Porth.

If you live outside of these areas please check with a receptionist that you live within the practice catchment area before completing your forms

You may print off a registration form, fill it out and either post or bring it into the surgery.  All your details will need to be entered onto the system before your first appointment.
